C-CDA
Consolidated Clinical Document Architecture
C-CDA is the US implementation guide that consolidates the most common CDA document templates (Continuity of Care Document, discharge summary, progress note) into one set.
US EHR certification requires it, so most documents exchanged between US systems today are C-CDA files, often carried inside a FHIR DocumentReference.
