In short: a good appointment starts with one page: why you are there, how it has changed, what you take, what you react to, and the three questions you most want answered. This tool builds that page from what you type and prints it. Nothing you write leaves your browser.

Five boxes
Main concern, course over time, medications, allergies, questions. The order doctors think in.

One printed page
Clean layout, large type, a line for the doctor's notes. Print it or save it as PDF.

Next time, automatic
In Anpheros Daily the same sheet is built from your record in one tap, with your lab results attached.
How to use it
- Main concern, in one sentence. "Headaches three times a week for a month, worse in the morning."
- How it has changed. Since when, how often, better or worse, what you have tried. If you kept a symptom diary, copy the trend here.
- Medications and supplements, with doses, including over-the-counter ones.
- Allergies and past reactions to medicines.
- Your three questions, most important first.
Press Print or save as PDF. The browser's print dialog lets you save the page as a PDF on any phone or computer.
What the sheet does not do
It does not suggest diagnoses or treatments, and it does not score or interpret anything. It puts your own words in the order a doctor can use. Our guide on how to prepare for a doctor's appointment explains why each box matters.
Frequently asked questions
Is the appointment checklist free?
Yes, with no account. It runs entirely in your browser.
Is what I type stored anywhere?
No. The text stays on your device until you print or copy it. If you want to keep it for the next visit, save the PDF or use Anpheros Daily, where appointments, documents and reports stay together.
Can I use it for a child or a parent?
Yes. Fill in the sheet for the person you accompany; the "about" line at the top lets you write their name and date of birth.
Can I add my lab results?
Not on this sheet; bring the reports or share them with the doctor. In Anpheros Daily the report for the visit includes results, measurements and medications from your record.
Does it work on a phone?
Yes. On iPhone and Android, "Print" offers "Save as PDF", which you can then show or send.
