EMR
Electronic Medical Record
An EMR is the digital version of one provider's paper chart: the clinical record kept inside a single practice or hospital, under that organisation's control.
It rarely leaves the organisation on its own, which is why EHR and PHR exist as the shared and the patient-held forms.
