Updated

A SMART Health Card is a signed, compact FHIR Bundle (typically immunizations or lab results) packed into a QR code that a verifier can check without contacting the issuer.

It was used widely for COVID-19 vaccination proof in the US and Canada and remains a general format for verifiable health facts.
