In short: this is a free vaccination record. Write each vaccine with the date, the dose number and, if your doctor gave you one, the date of the next dose. The next doses appear in date order. Print the record or save it to your free Anpheros record (family profiles with Plus or Max). It keeps your dates; your doctor or national schedule decides which vaccines you need.

This is a log of the values you enter. It does not interpret them or give medical advice. Talk to your doctor about your results and your targets.

This record does not replace an official vaccination certificate.

Every vaccine, with its date

Copy them from your vaccination book, certificates or a clinic letter.

The next doses, in order

Write the next dose date your doctor gave you; the list keeps them in date order.

For the whole family

With Plus or Max, each child and parent can have their own vaccination record in your account.

How do I use this vaccination record?

  1. Add a row for each vaccine: the name as written in your book or certificate, and the date.
  2. Add the dose number if it is part of a series, and the next dose date if your doctor gave you one.
  3. Use the note for the batch number or the clinic, if you want.
  4. Print the record, or save it to your free account; a profile for each family member is on the Plus and Max plans.

Where do I find my past vaccines?

Your vaccination book, school or travel certificates, your GP's records or your country's patient portal usually have them. If a date is missing, ask your doctor; do not guess dates. Our guide to vaccination records for the whole family explains where to look and how to keep them.
