Health records and AI
The kinds of health records and who controls them, why patient-held records matter, and what AI needs from medical data: structure, coding, consent and safety.
An electronic health record (EHR) is the record a care provider keeps and can share across settings, an electronic medical record (EMR) is usually one practice's clinical record, and a personal health record (PHR) is the record the patient controls. AI tools are only as reliable as the data under them: coded, structured, FHIR-based records with known provenance are what make AI on medical data useful and auditable.
| EHR | Provider record, shareable across care settings |
|---|---|
| EMR | One organisation's clinical record |
| PHR | Patient-controlled record, from many sources |
| What AI needs | Structured, coded data with provenance and consent |

What does AI need from medical data?
- AI on medical data: the map for buildersWhat it takes to use large language models and other AI on health records responsibly: where the data comes from, how models reach it (context APIs, MCP, RAG), how patients stay in control, how privacy is protected, how outputs stay traceable, how systems are evaluated, and which rules apply.
