Short answer: Poland's national e-health runs on the P1 platform (Elektroniczna Platforma Gromadzenia, Analizy i Udostępniania Zasobów Cyfrowych o Zdarzeniach Medycznych), operated by the Centrum e-Zdrowia (CeZ) under the Ministry of Health. It delivered, in quick succession, e-recepta (electronic prescription, mandatory for all prescribers since January 2020), e-skierowanie (electronic referral, mandatory since January 2021), the e-zwolnienie (electronic sick leave, with the social insurance), the national register of medical events (zdarzenia medyczne) that every provider must report, and the exchange of electronic medical documentation (EDM) indexed in P1. Citizens use the Internetowe Konto Pacjenta (IKP) and the mojeIKP app to see prescriptions, referrals, visits, documents and vaccinations, and to manage consents. Providers connect with their software through CeZ's integration environment.
The services
Poland's P1 platform delivers national e-health services rather than one record: e-recepta electronic prescriptions, e-skierowanie referrals, e-zwolnienie sick-leave certificates, the mandatory register of medical events, the exchange of electronic medical documentation, central e-Rejestracja booking, a vaccination record, and the IKP patient account with its mojeIKP app used by tens of millions of citizens.
| Service | What it does | Status |
|---|---|---|
| e-recepta | Prescriptions issued electronically, dispensed in any pharmacy by code or ID; data kept centrally | Mandatory since 2020; near-universal |
| e-skierowanie | Referrals to specialists, hospitals and diagnostics, redeemed at any provider | Mandatory since 2021 |
| e-zwolnienie (e-ZLA) | Sick-leave certificates sent to the employer and ZUS electronically | Mandatory since 2018 |
| Zdarzenia medyczne | Every medical event (visit, admission, procedure) reported to P1 with core data | Mandatory since 2021 |
| EDM exchange | Electronic medical documents (discharge letters, reports, lab results) indexed in P1 and retrievable by authorised providers | Rolling out by document type |
| e-Rejestracja | Central appointment booking | Expanding |
| Vaccination record | COVID-19 and routine vaccinations in IKP | Live |
| IKP / mojeIKP | Citizen portal and app: prescriptions, referrals, visits, documents, consents, proxies for children and relatives | Live, tens of millions of users |
How it is built
Poland's P1 is a central platform of registers and services, not a central document store: medical documents stay in providers' systems while P1 holds the index, the medical events data, and the prescriptions and referrals themselves. Citizens log in to IKP with PESEL-linked identities such as Profil Zaufany or mObywatel; software connects through CeZ's integration environment.
P1 is a central platform with national registers and services, not a central record of documents: medical documents stay in the providers' systems (or regional repositories) and P1 holds the index and the medical events data, plus the prescription and referral data themselves. A provider with a care relationship (confirmed by the patient's consent in IKP or by the event itself) queries P1 for the index and retrieves the document from the source. Identity: the PESEL national number for citizens, Profil Zaufany, bank identities or mObywatel for logging in to IKP; professionals use their registration numbers and certificates issued by CeZ. Software connects to P1's integration environment with certificates and must pass CeZ's conformance tests per service.
Standards and codes
Poland's P1 uses HL7 CDA R2 documents following the national PIK HL7 CDA implementation guide for prescriptions, referrals, discharge letters and reports, and HL7 FHIR R4 with Polish profiles from CeZ for medical events and new APIs. Codes are the Polish ICD-10 edition, ICD-9-PL for procedures, the national medicines register with ATC, LOINC and SNOMED CT.
- Electronic documents: HL7 CDA R2 following the Polska Implementacja Krajowa HL7 CDA (PIK HL7 CDA), the national implementation guide, for prescriptions, referrals, discharge letters, lab and imaging reports and more.
- Medical events and new APIs: HL7 FHIR R4 with Polish profiles published by CeZ.
- Codes: ICD-10 (the Polish edition) for diagnoses, ICD-9-PL for procedures, the national medicines register (Rejestr Produktów Leczniczych) with EAN codes and ATC, LOINC for laboratory in new specifications; SNOMED CT is licensed nationally since 2021 for use in the national system.
For vendors
A software vendor connects to Poland's P1 by integrating with CeZ's integration environment for e-recepta, e-skierowanie and medical event reporting, following the published specifications, test environments and conformance procedure for each service. Vendors from abroad follow the same path; patient-facing apps have no direct API to IKP and rely on the citizen's downloads.
For vendors from abroad, Polish-language documentation and the national profiles are the practical barrier; the citizen downloads or shares documents from IKP for any third-party app.
Cross-border and the EHDS
Poland exchanges ePrescriptions and the patient summary with other EU countries through MyHealth@EU via its national contact point, among the earlier adopters for ePrescription. Under the EHDS, P1's priority categories will be served in the European exchange format; the central architecture makes Poland comparatively well placed.
For a person with an IKP
A person in Poland with an Internetowe Konto Pacjenta (IKP) logs in to the portal or the mojeIKP app to see prescriptions and referrals, redeem codes, view visits and vaccinations, download documents, grant proxies and set consents. Downloaded documents can be kept in a personal record such as Anpheros Daily for care abroad.
Log in to IKP or mojeIKP to see prescriptions and referrals, redeem codes, view visits and vaccinations, download documents, grant proxies and set consents for data access. A record such as Anpheros Daily can store what you download, in FHIR with provenance, for care abroad; see ePrescription across EU borders for how your prescriptions travel.
National systems change often. This page describes the situation as published by the operators linked under Sources, last reviewed in October 2026; the operator's documentation is the authority for connection requirements.

Frequently asked questions
Is Poland's system a central health record?
Partly. Prescriptions, referrals and medical events are held centrally in P1; medical documents stay with providers and are indexed in P1 for retrieval. IKP shows the citizen all of it in one place.
How do citizens log in?
With Profil Zaufany (the national trusted profile), a bank identity, the mObywatel app or an e-ID card. Parents and carers can access records of children and relatives through proxies set in IKP.
Which standards does P1 use?
HL7 CDA R2 with the Polish national implementation guide for documents, and HL7 FHIR R4 for medical events and newer APIs, with ICD-10 for diagnoses and the national medicines register for prescriptions.
Can I use a Polish e-recepta abroad?
In countries connected with Poland through MyHealth@EU for ePrescription, yes, at participating pharmacies. Elsewhere you need a paper prescription or a local one.
