FHIR CarePlan resource

CarePlan is the FHIR resource that describes how one or more practitioners intend to deliver care to a patient over a period: the activities planned, the conditions addressed and the goals aimed for.

It is used for chronic-condition plans, remote monitoring programmes and discharge plans, and links to the CareTeam, Goal and ServiceRequest resources that make the plan concrete.
