Continuity of Care Document

Updated

CCD is the C-CDA document type that summarises a patient's current health record: problems, medications, allergies, results, procedures and immunisations, each in a coded section.

It is the summary most US EHRs export when a patient moves between providers or downloads their record, and the International Patient Summary plays a similar role elsewhere.
