Unified Medical Language System
The UMLS is an NLM resource that links over 200 vocabularies, including SNOMED CT, LOINC, RxNorm, ICD and MeSH, by grouping their codes under shared concept identifiers (CUIs).
It is free with a licence and is widely used for mapping between code systems and for natural language processing of clinical text.
