Gravity Project
HL7 Gravity Project (social determinants of health)
The Gravity Project is an HL7 FHIR Accelerator that develops terminology and FHIR standards for social determinants of health, such as food insecurity, housing instability and transportation needs.
Its codes are published in LOINC, SNOMED CT and ICD-10-CM, and its SDOH Clinical Care implementation guide describes screening, problems, goals and closed-loop referrals to community services.
