Your blood tests are a PDF in an e-mail from 2023. The cardiologist's letter is in a drawer. The vaccination booklet is at your parents' house, and the name of the antibiotic you reacted to is "something with a C". Every clinic keeps its own record of you, and nobody keeps the whole one. A medical records app is the place where you do.

Scan any document
Lab reports, prescriptions, letters, imaging reports: a photo or a PDF, and the app reads the text.

Medications, allergies, vaccines
The list every doctor asks for first, coded to the standards hospitals use.

You decide who sees it
Access for a doctor is approved by you, for a period you choose, and logged.
A medical record app for patients, not for hospitals
Hospitals and clinics run electronic medical record systems, and each one knows only what happened inside its walls. When you leave, you get a printout, a PDF or a login to yet another portal. A personal medical record app is the opposite: one record that belongs to you, follows you from clinic to clinic and country to country, and is still there in ten years when the clinic has changed systems twice.
Anpheros Daily is built as that record. You add things once, from your phone, and the app keeps them in order by person, by date and by type.
What goes into your digital medical record
- Documents. Lab results, prescriptions, medical letters, investigation and imaging reports. Scan them page by page or upload the file. The app reads the text, so a lab report becomes values with reference ranges, not just a picture.
- Medications. Scan the box and the app recognises the medicine, the active substance and the strength. Daily doses are ticked off on the Home screen.
- Conditions, allergies and vaccines. The things a new doctor needs in the first minute, and the ones that go on your emergency card.
- Symptoms and measurements. Blood pressure, weight, glucose, sleep, how you feel, each with its time, on one timeline.
- Doctors and appointments. Who looks after whom, with the documents from each visit attached to the appointment.
You do not have to fill everything in on day one. Most people start with the medication list and the last lab report, and add the rest as it arrives.
Electronic health record app: the standard underneath
Everything is stored as HL7 FHIR R4, the format hospitals and national health systems use, with standard codes: LOINC for lab tests, ATC for medications, ICD-10 for conditions. That is what makes an electronic health record app different from a folder of photos: the data can be compared across years and laboratories, and read by any system that speaks the standard. Our guide explains what FHIR is and why "yours" should mean "exportable".
And it is exportable, at any time: as a PDF, as FHIR JSON, or as an International Patient Summary, the standard summary European hospitals can read.
Who sees your record
Nobody, until you say so. The emergency QR code on your phone shows a rescuer only the basics: who you are, blood group, allergies, medications, emergency contact. A doctor or a clinic that works with Anpheros can ask for access to your history; you approve what they see and for how long, and you can revoke it. Every read is in your access log. You can also send a doctor a time-limited link to a report before a visit.
Your data is stored in the European Union, under the GDPR. Anpheros shows no ads and sells no data.
Medical records app for iPhone, Android, Mac and the web
The same account works on iPhone and Android, on Mac, and in the browser at app.anpheros.com on any computer. What you scan on the phone is on your Mac a second later. The app comes in 20 languages, and because the record is coded to international standards, a doctor abroad reads your medications and results in their own language.
For the whole family
On the Plus and Max plans you add profiles for your children, your parents or anyone you look after, each with its own record, its own colour and its own doctors. See the family health record.
How to start
- Install the app on iPhone or Android, or open app.anpheros.com.
- Create an account and pick your language.
- Scan your last lab report or the box of a medication you take. That is your first entry.
The Basic plan is free and includes your medical record, documents, medications, symptoms and measurements, with no time limit. Plus and Max add family profiles, data from your watch, HD scans and the AI assistant that explains your documents and prepares your questions for the doctor.
Frequently asked questions
Is there an app for my health record?
Yes. Anpheros Daily is a personal health record app: you keep your documents, medications, lab results, allergies, vaccines and measurements in one place, on your phone and in the browser, and show them to any doctor you choose.
Is the medical records app free?
Yes. The Basic plan is free, with no card and no time limit, and includes the record itself: documents, medications, symptoms, measurements and the emergency card. Plus and Max add family profiles, data from your watch, HD scans and the AI assistant.
Is there a medical records app for Android and for iPhone?
Yes, the same app, with the same account, on Android and iPhone, plus a Mac app and the browser version at app.anpheros.com. Everything syncs between them.
What is the difference between this and the hospital's electronic medical record?
The hospital's record is the hospital's: it covers what happened there and you cannot take it with you. A personal medical record app is yours: it collects what every clinic, laboratory and doctor gave you, across years and countries, and you decide who reads it.
Can I import my records from a hospital or a national portal?
You scan or upload the documents you have, PDF or paper, and the app reads them. Laboratories and clinics that work with Anpheros send results directly into your record. There is no automatic import from national portals yet; the EU's EHDS rules are meant to make that possible, and our guide on your medical data in the EU explains where that stands.
Where is my data stored, and can I take it out?
In the European Union, in the standard HL7 FHIR format. You can export the whole record as PDF or FHIR JSON at any time, or generate an International Patient Summary, and you can delete it with your account.
Can my doctor see my record?
Only with your consent. You can send a time-limited link to a PDF report, or, if the doctor or clinic works with Anpheros, give them access to your record for a period you choose, revocable at any time, with every read logged.
Can I keep medical records for my family?
Yes, on the Plus and Max plans. Each child, parent or partner gets their own profile and record in your account, and if they have their own account you can share with them what you keep.
